Community Partnership Guidelines

  • All forms must be submitted 30 days prior to your organization’s desired request.

  • Organizations seeking partnership must be a non-profit entity.

  • Organizations must submit a request through this online form. Due to the volume of donations requests that we receive, we only accept requests through this online form.

  • The organization and/or event must be located in Ascension Parish or general community area. (If you are not located in this community, please reach out to the location closest to your location and/or event.)

  • Monetary donations or gift cards are not available.

Expression of Interest Forms

Donation Request Form: You are requesting a donation for an event/fundraiser you already have established.

Fundraiser Request Form: You are interested in partnering with us for a Chick-fil-A approved fundraising opportunity.
